Requests for names of fallen warriors to be memorialized on the wall must include:
1. Veterans Name
2. Branch of Service
3. Date of Birth
4. Date of Discharge
5. Confirm that they were treated for PTSD
6. Confirm death by suicide
7. Relationship of the person making request
8. Contact information including name, address, phone number, and email address.
9. Photo of the Veteran
10. Number of people attending Memorial Service. 
Include all the above information to either the mailing address or the listed email below.
